Worship Service Reservation

Please note the below form is for making a reservation to attend the next Sunday's worship service for those with an unknown vaccination status.  As per Victorian government regulations, we can only accept 30 people per venue when the vaccination status of attendees is unknown.  Completing the reservation form does not automatically confrm the reservation.  A church administrator will be in contact to confirm the reservation.
